A Travel Consent Letter is typically required if a minor child travels abroad with only one parent, another adult (a relative or an unrelated adult), or alone. Travel Consent Forms can be requested from the airlines your child is flying with, or you can draft your own letter. A Travel Consent Letter must contain the following information:

- information about consenting parents/legal guardians (their full names, passport information, permanent address, contact information)
- information about a traveling minor (his/her full name, passport details, permanent address)
- information about the trip (dates of travel, flight number, detailed itinerary, and addresses where a minor will stay on the trip, contact details for his/her place of stay)

A Travel Consent Letter must be prepared in English and can be duplicated in a language of a destination country if needed. If one parent gives consent to another parent or adult to travel with a child, that parent must sign the Travel Consent Letter. However, if both parents are consenting to their child’s travel, both must sign a Travel Consent Letter in the presence of a notary public. A Travel Consent Letter must be notarized by a licensed notary public, bear a notary stamp, a signature, a date, and a notarial affidavit if required by the state. When you choose a notary public make sure you select an experienced notary with a valid license and knowledge of notarial laws in his/her state. Incorrectly notarized Travel Consent Letters will not be accepted for apostille processing, which may result in trip delays and unplanned extra costs.

When your Travel Consent Letter is notarized, you can mail it to our office for apostille processing. An apostille on a Travel Consent Letter is always processed by the same state where the document was notarized. For example, if your Travel Consent Letter was notarized in Georgia, an apostille on your document will also be processed by the State of Georgia.
